How will you promote and/or advertise this event? PLEASE NOTE: Parkway Plaza Mall requires all persons and organizations that perform or hold an event in the mall to provide a certificate of insurance, listing Parkway Plaza Mall with the coverage amounts outlined below.
• Your insurance company must prepare the certificate of insurance showing that you or you organization has said coverage while performing, hosting an event, etc. in/on the grounds of Parkway Plaza Mall. • Certificate of insurance must be on file a week prior to date of performance/event. • Insurance requirements are: 1,000,000.00 general liability and $500,000.00 property damage and the following names must be listed as 'additional insureds’: Ershig Properties, Inc. dba/Parkway Plaza Mall and Parkway Plaza Mall Merchants’ Association, Inc. • Certificate holder’s address is: Parkway Plaza Mall Office, 455 Madison Square Ave., Madisonville, KY 42431.
Event organizers will be responsible for table skirts and covers, phone line, PA/microphone/CD or cassette player if needed. We do require that tables are skirted and must be covered to the floor on all sides, to be representative of the best efforts of both event organizers and Parkway Plaza Mall.
